New York City is one of the largest and most popular fan-made cities for Midtown Madness 2. It is based on the real-life New York City.

It was originally uploaded to Midtown Madness 2 eXtreme on August 29th, 2006, by Yallis and Richard P., as a port of the city used in another one of Angel Studios' games, Midnight Club: Street Racing. A new version, named New York City Improved 1.2, was released on September 22nd, 2010, with the help from SuperSecret. Both variations of the city include their own unique set of traffic cars and are very prone to crashes, especially if both traffic and high-detail cars are used. Also, four police vehicles, a Meter Maid electric cart, a 1977 Chevrolet Impala, a Chevrolet Suburban and an NYPD edition of the Ford Mustang Cruiser, are included for the player. The large American city in Midnight Club: Street Racing, and consequently, the MM2 mod, took place entirely on Manhattan island, in the center of the city, where most of its famous landmarks are located.

- The original World Trade Center can be seen. It shows the original twin towers because the game was released prior to 9/11.
- The Manhattan Island has only two bridges and the player cannot drive on them due to the fact that is inaccessible.
